Dr. Sachin Karande is a Columbia University–trained dental physician and founder of Karande Biological Dentistry in Stafford, Virginia, where he is helping redefine the future of dentistry through a systems-based approach that integrates airway science, regenerative biology, and whole-body physiology.
In Dr. Karande’s view, dentistry is entering a profound transition.
For more than a century, the profession has largely focused on repairing damage. The next era will focus on understanding why disease develops in the first place—and how restoring structure, breathing, and biological balance can support the body’s capacity to heal.
His work explores the intersection of craniofacial structure, breathing, inflammation biology, and systemic health.
Dr. Karande earned his Doctor of Dental Surgery from Columbia University College of Dental Medicine, where his training emphasized systemic pathology, inflammatory mechanisms, and the oral–systemic connection. Columbia’s medically rigorous curriculum cultivated a diagnostic philosophy grounded in physiology rather than procedure-based dentistry.
He later pursued advanced education at Tufts University School of Dental Medicine, completing training in Integrative Biological Dental Medicine, Oral and Maxillofacial Pain, and Temporomandibular Disorders (TMD). This work reinforced a clinical model that aligns structural correction with immune regulation, regenerative healing, and long-term physiologic stability.
Airway as the Foundation of Health
A central focus of Dr. Karande’s clinical work is airway-centered dentistry.
As a member of the American Academy of Dental Sleep Medicine, he studies how craniofacial structure and jaw position influence breathing, sleep physiology, autonomic regulation, and systemic recovery.
Breathing governs far more than oxygen intake.
It influences nervous system balance, cardiovascular function, inflammation control, hormonal regulation, and cellular repair.
When airway stability is compromised, the body often enters a cycle of chronic physiologic stress. Dr. Karande’s approach uses advanced 3-dimensional CBCT imaging, neuromuscular analysis, and occlusal diagnostics to identify structural patterns that affect breathing and restorative sleep.
In this model, airway stability becomes a biological foundation for healing and long-term health.
Molecular Research in Implant Biology and Inflammation
Alongside his clinical work, Dr. Karande has participated in research exploring the molecular biology of peri-implant inflammation and tissue regeneration.
His studies investigating antimicrobial photodynamic therapy (aPDT) demonstrated that this treatment can significantly increase epidermal growth factor (EGF) gene expression, suggesting a regenerative pathway that may support healing in peri-implant mucositis.
Further molecular investigations have identified elevated expression of inflammatory signaling molecules such as IL-13 and CCL17 in peri-implant mucositis, providing insight into early immune responses that precede implant complications.
Additional work examining hypoxia-inducible factor HIF-1α has suggested its potential role as an early biomarker of peri-implant inflammation, revealing a strong regulatory relationship with the anti-inflammatory cytokine IL-10.
Beyond implant biology, Dr. Karande has also contributed to infectious disease research examining the detection of SARS-CoV-2 and opportunistic bacterial pathogens in the oral and respiratory tract, highlighting the oral cavity’s critical role in microbial surveillance and systemic health.
Together, these investigations reflect a broader interest in how molecular signaling, microbial ecology, and inflammatory biology influence oral and systemic disease processes.
Regenerative and Biological Treatment Models
Dr. Karande is an Accredited Member of the International Academy of Oral Medicine and Toxicology (IAOMT) and affiliated with the International Academy of Biological Dentistry and Medicine (IABDM).
His clinical protocols integrate regenerative techniques such as:
• platelet-rich fibrin (PRF)
• ozone-assisted surgical therapies
• ceramic implant systems
• immune-compatible biomaterials
These approaches aim to support tissue regeneration, immune balance, and biologic compatibility within modern dental treatment.
Dr. Karande has also participated in international regenerative research collaborations in Brazil, further expanding his work in bone regeneration and biologically guided healing strategies.
Integrating Ancient Wisdom and Modern Science
Beyond contemporary biomedical science, Dr. Karande draws inspiration from Ayurveda, the ancient science of systemic balance.

Earlier in his career, Dr. Karande served as Dental Director at Western State Hospital, Virginia’s state psychiatric and medical facility, where collaboration with physicians and psychiatrists reinforced the importance of understanding oral health within the broader context of systemic physiology.
Dr. Karande joined the Bye-Ol Dentistry movement because he believes the profession stands at an inflection point.
Airway science, regenerative medicine, inflammation biology, and systems-based diagnostics are beginning to reshape dentistry’s role within healthcare.
